Insurance Reimbursement
I am in network with
- Aetna
- Anthem
- Blue Cross & Blue Shield
- Cigna
- Traditional Medicare; selected Medicare Advantage plans
Please note. Insurance companies contract with psychotherapists to provide sessions at the companies’ contract rates. The company, not I, determines the nature of coverage and the coinsurance and copayments. As a participant in your insurer’s panel of providers, I agree to accept the insurer’s contract rates, and I must bill the insurer directly for services.
The amount of your copay or coinsurance that is payable at each session will depend on your contract with the company, including your deductible and/or health savings account (HSA), if any. I do not determine the amount of my reimbursement, nor do I determine the size of your copay or coinsurance, if any.
You are responsible for determining coverage and benefits under your particular policy. For information, call the behavioral health, or mental health/substance abuse (MH/SA), telephone number on the back of your insurance card.
Warning
Insurance companies will not reimburse for the services I provide unless I assign you a medical diagnosis. Please note that your insurer may also refuse to authorize services unless I supply other clinical information about you in writing.
Neither you nor I have control over your personal health information once it goes into cyberspace. Clinical information and diagnoses, once supplied to third parties, may be disseminated electronically and may affect your ability to find a job or to get life insurance, workplace promotions, or other benefits. To protect your privacy, you may well prefer not to use medical insurance to pay for mental health benefits.
Policies
To cancel or reschedule an appointment, you must give me 24 hours’ notice during business hours to avoid being charged the full session fee. The missed appointment fee is not just the copay or coinsurance but the full reimbursement rate: if you are seeing me under insurance, please bear in mind that insurance companies do not pay for missed sessions, and you will be liable for the full contractual rate at which I am reimbursed.
During our work together please tell me if you are not finding our sessions helpful so that we can discuss your concerns and adjust your treatment accordingly. I rely on your feedback.
I would like to support you throughout your work in therapy, including termination. If you decide to end our sessions, please tell me ahead of time so that I can help you leave therapy with a feeling of completion.
As a licensed therapist, I protect the confidentiality of your communications with me. I release information about our work to others only with your written permission or if I am required to do so by law. In some situations I am legally obligated to breach your confidentiality in order to protect others from harm:
- If I have information indicating that a child or elderly or disabled person is being abused, I must report that to the appropriate state agency.
- If a client is an imminent risk to him/herself or makes threats of imminent violence against another person, I am required to take protective action.
In either case, I will invite you to discuss the matter with me before I proceed.
